How to Register with the Practice
We provide medical services a wide area within Old Meldrum, Fyvie and the surrounding areas.

A map of our practice area is shown below. If you live within this area, you are entitled to register with the practice. If you live out with our practice area or move home out with this area then you will be required to register with another practice. Please advise us of any change of address, form available below:
These forms need to be printed off, completed and handed into the surgery with proof of ID and proof of address
As your medical records can take some time to arrive from your previous GP, we ask all new patients to complete a questionnaire to inform us of essential health information, regular medications and allergies. Please complete this as accurately as possible so that we can give you the best possible care as soon as you register with us.
